Poster: GM-HFIT – Improving the management of heart failure in primary care

Heart failure affects around 900,000 people with 60,000 new cases annually and accounts for 2% of NHS inpatient days and 5% of emergency admissions to hospital. The purpose of this project is to improve the detection, management and care of heart failure within primary care in Manchester.

A presentation providing information on improving the management of heart failure in primary care, GM-HFIT and a poster detailing the purpose, methods, results and conclusion of the GM-HFIT project were presented at the HSRN Symposium NHS Confederation Conference in June 2012.
